It will apply to those who are booking an appointment for same-day service from 6 April 2015 (not to those who book before that, your appointment can be within 6 weeks from the date of booking). Those applying outside the UK will have to pay it if submitting an online form and paying a UKVI fee from 6 April 2015 (but not before that, even if your visa centre appointment is later).  

New Immigration Rules from 6 April 2015: Tier 1 General extensions will no longer be possible, apply by 2 April if you need one! Home Office official fees will be increased again - apply in March to save money, subject to eligibility. Spouse/Partner in-country visa application fees go up by £355 while ILR fees for all - up by £400! A business plan will be made compulsory for initial applications for a Tier 1 Entrepreneur visa, at the moment it is advisable but not compulsory.
